Does stress actually shorten your life?

At age 40, the difference between the best and worst answer here is about 3.2 years of life expectancy — 3.3 for men and 3.0 for women. That is the gap between “rarely stressed” and “running on empty”, holding everything else constant.

Stress gets treated as a feeling, which is why it is routinely dismissed as a health factor. The cohort evidence treats it as an exposure — something with a dose, a duration and a measurable effect on the cardiovascular system — and on that reading it earns its place in the model.

Does it matter less as you get older?

Yes, and by a lot. A hazard ratio applies to the years you have left, so the same answer costs less in absolute terms the later you are asked it. That is an argument for changing it early, not for concluding it no longer matters.

What the evidence actually shows

Kivimäki and Steptoe's review in Nature Reviews Cardiology drew together work covering hundreds of thousands of workers and found that chronic job strain carried roughly 10 to 40% higher risk of coronary heart disease and stroke, with a clear gradient by exposure duration. The effect persisted after adjustment for smoking, weight and activity — it is not simply a proxy for people under stress living badly.

What to do about it

Chronic stress is not a mood, it is a cardiovascular exposure. Anything that reliably drops your baseline — therapy, boundaries, a different job — is a longevity intervention.

Where this number is weak

Stress is self-reported and its measurement is far less standardised than a cigarette count or a BMI. The model gives it a deliberately narrow range as a result. What it cannot capture at all is that the same objective circumstances land very differently on different people.

More generally: these are observational cohorts, which show association rather than proof of cause. The full methodology sets out every simplification the model makes.
